#1:  2014-15 Sports Physicals

Every student-athlete must have a completed sports physical on file with the Athletic Trainer before beginning a sport.  Once this is on file, a recertification form must be completed for each subsequent sport throughout the school year.  Only physicals dated/verified after June 1, 2014 will be valid.
We offer Sports Physicals at the school TWO DATES for each school year: one in June and one in August. 
The physical is then valid for the entire school year.
Sports Physicals for the 2014-15 school year will be by appointment only on the following dates:
June 5th  5-9pm (deadline for making appt is June 4th, 4pm)
July 28th  5-9pm (deadline for making appt is July 24th, 4pm)

Bring physical packets (completed by parent/guardian) and $22 cash or check made out to DTSD. 
Physicals will take place in the High School Nurse's office.  Please enter through the main doors.

Please complete the packet and take it to your family doctor to complete, or go to one of the following Urgent Care walk-in clinics and ask for a sports physical:


# 2:  ImPACT Baseline Test Sessions

Middle and High School student-athletes in 7th, 9th and 11th grades as well as new athletes to the sport will need to take a baseline concussion test prior to participating in the first practice for the following higher-risk sports: 
Football + Soccer + Field Hockey + Cheerleading + Volleyball 

Testing sessions for 2014 fall sports took place in May/June.  If you still need to take the test, please contact one of us below...
