Every student-athlete must have a completed sports physical on file with the Athletic Trainer before beginning a sport. Only physicals dated/verified after June 1, 2014 will be valid. A recertification form is needed for each subsequent sport or if the physical was performed more than 6 weeks prior to the beginning of the sports season.
Physical packets are available in the MS Office, HS Athletic Office, or downloaded and printed HERE.
We offer Sports Physicals at the school TWO DATES for each school year: one in June and one in August.
The physical is then valid for the entire school year.

**More on the Recertification Form:
1. It lets the Sports Medicine staff know to pull the physical and assign it to the correct sport's folder for that season.
2. It lets the Sports Medicine staff know if there have been any new injuries that need to be addressed.
It does NOT need to be filled out by a doctor UNLESS the athlete has
seen a doctor since their original physical date. Then, the doctor
needs to clear them for return to participation by signing the back of
the recertification form.
