Sports Medicine

#1:  2014-15 Sports Physicals

 
Every student-athlete must have a completed sports physical on file with the Athletic Trainer before beginning a sport.  Once this is on file, a recertification form must be completed for each subsequent sport throughout the school year.  Only physicals dated/verified after June 1, 2014 will be valid.
 
We offer Sports Physicals at the school TWO DATES for each school year: one in June and one in August. 
The physical is then valid for the entire school year.

Please complete the packet and take it to your family doctor to complete, or go to one of the following Urgent Care walk-in clinics and ask for a sports physical:
                           
 
  

#2:  Emergency Information

 
To simplify tracking email addresses and problems logging into the system, we are switching to an Emergency Card for this year.  Please obtain this form from your coach the first week of practice or print and complete HERE.
 
EMERGENCY CARDS MUST BE COMPLETED AND RETURNED TO THE COACH BEFORE FIRST SCRIMMAGE OR ATHLETE WILL NOT PARTICIPATE!
 

# 3:  ImPACT Baseline Test Sessions

 
Middle and High School student-athletes in 7th, 9th and 11th grades as well as new athletes to the sport will need to take a baseline concussion test prior to participating in the first practice for the following higher-risk sports: 
Basketball  +  Wrestling  +  Cheerleading
